  • Criminal record certificate

    Issued by competent authority in country of nationality

    Include all criminal records within home country

    If criminal record confirmation system within country of nationality inadequate, can substitute with certificate from internal affairs agency having jurisdiction over residence

    Issued within 3 months from visa application date

  • Health status confirmation

    Handwritten by visa applicant

    Record infection status for tuberculosis, hepatitis B, syphilis, etc., drug use experience, mental illness treatment experience, etc.
